Position Description:
Orange County, CA
Located in Orange County, CA at the site of the designated client, the Clinical Well-being Program Coordinator (CWPC) will work collaboratively with the designated client’s Wellness team and WebMD Health Coaching team to provide chronic condition management programming to eligible participants as well as raise awareness of the onsite program and the WebMD products and services that enable it. The programming will support the client’s overall strategic plan. The CWPC will work with and be a resource for the individual employer units in their health promotion activities.
The CWPC is responsible for client satisfaction, retention and reference-ability. S/he will act as customer advocate, as well a point of contact with senior Customer stakeholders to increase participant engagement in all health and wellness resources provided by the County of Orange. They can provide thoughtful leadership and support program planning and impact assessment, as well as coordination and promotion of service.
CWPC’s proactively identify opportunities for service delivery enhancement and improvement. As opportunities are identified, they work with other Client Service staff, Account and Product Management to improve processes, and develop new solutions.
In this role you will:
- Create health education and promotion activities and classes for populations with diabetes and/or obesity and other chronic conditions, such as but not limited to, high blood pressure, high cholesterol, asthma, and COPD.
- Maintain current knowledge of evidence-based wellness concepts and guidelines related to exercise, nutrition, weight loss, tobacco cessation, blood pressure control, and cholesterol management, and diabetes management.
- Utilize established tools & protocols provided to assess participant health status, and to identify health risks
- Educate participants about health management concepts using evidence-based guidelines
- Assist participants with developing plans and goals to mitigate their health risks
- Leverage evidence-based coaching techniques (such as motivational interviewing, active listening and decisional balance) to elicit behavior change
- Assure accurate documentation of participant progress to support outcomes evaluation and to assure continuity of care
- Maintain specified health credentialing, including licensure, certification and ongoing continuing education
- Assist in the production of aggregate reports which incorporate the health education calls made to client's participants
- Develop and maintain schedules necessary to support the successful execution of client health education programs
- Conduct body composition testing, and follow progress of client to ensure they meet or exceed their goals
- Integrate nutrition into other facets of existing wellness programs
- Work closely with the client Program Manager, members of the Wellness team, to include WebMD, to develop calendar, schedule and deliver of events tied to on and off campus programs and activities for maximum engagement and nutrition education
- Perform other, varied job-related duties as assigned
Qualifications, Skills and Experience:
- Must be Registered Dietician (RD)
- Certified Diabetes Educator (CDE) is required for this role
- Ability to maintain active, unprohibited RD licensure in the State of California
- Minimum of 2-5 years Wellness Coordinator/Health Coaching or similar client-facing experience required (5 years preferred)
- Demonstrated ability to successfully manage multiple relationships with multiple client
- stakeholders in a dynamic environment.
- Valid driver’s license is required. Must be willing to regularly travel between office locations around Orange County.
- Professional presence and technical credibility to interact with senior-level customer contacts.
- Familiarity with MS Office suite including Word, PowerPoint, and Excel. Experience with or willingness to learn Adobe Suite and Canva.
- Have excellent communication, organizational, multi-tasking, presentation and computer skills

COMP: $80,000 to $85,000
