What You Will Do:

The Clinical Team Manager supervises Home Health staff in both the visiting and documentation process.
Responsibilities include:
Assist in managerial functions of the agency. Mentor and counsel staff as appropriate, which includes OASIS documentation and skill proficiency. Seek counsel from Director of Clinical Operations for all personnel corrective actions. Complete timely field evaluations and annual performance appraisals for staff. Interview, hire and assist with orientation and ongoing education for staff. Assist with action planning and performance initiatives of the agency.

What You Will Bring:

Education

Bachelors of Nursing (BSN) from accredited school of nursing required or Bachelors, Master or Doctoral degree in Physical Therapy or Occupational Therapy required

Experience
Minimum 1 year of current experience in Home Health Supervision preferred
Minimum 2 years of staff nurse experience in home health care preferred

Certification & Licensures
West Virginia and/or Virginia licensed Registered Nurse or licensure eligible required*
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required**
Valid driver's license and reliable transportation required
*Licensure dependent upon geographical area assigned
**New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

Qualifications
Must be knowledgeable and competent to provide services for pediatric, adult and geriatric populations with various diagnoses.
Able to prioritize work demands.
Good organizational skills with attention to detail.
Be able to delegate responsibility and hold staff in department accountable.
