What You Will Do:
The Transition Coordinator - LPN is responsible for managing the discharge of patients from WMC as appropriate to all post acute/long term care facilities within and outside of Valley Health, as well as from non Valley Health referring facilities. This position works closely with the staff at WMC as well as the staff at each post acute/long term care facility coordinating the process from referral to admission. The Transition Coordinator will be actively involved with patient assessments and interviews, communicating with family members, and arranging tours of the respective facilities as necessary. The Transition Coordinator will work with the marketing department in the development of referrals from and outreach to other facilities outside of Valley Health.
What You Will Bring:
Graduate of an accredited school of Nursing.
Previous post acute/long term care admissions and marketing experience preferred Previous experience in post acute care (i.e. rehabilitation nursing, insurance case management or catastrophic case management) preferred.
Certification & Licensures
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required *
Licensed as a Licensed Practical Nurse in the State of Virginia within 90 days of hire. Valid driver's license and automobile.
Competent to successfully interact with patients according to their special needs which may apply based on the age of the patient served
