What You Will Do:
The MDS Coordinator is responsible for assisting with the coordination and implementation of activities of the Long-Term Care unit, including assistance with completion of MDS; provides skilled and intermediate resident care in accordance with hospital policies and procedures; functions in an advisory capacity to Administration in evaluating proposed procedural changes as they relate to patient care; assists with directing the implementation of, and ensures compliance with, Standards of Nursing Practice that promote optimum health care delivery; assists the Director of Nursing.
Duties include, but are not limited to: Coordinates RAI process assuring the accuracy, timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. Conducts the nursing process - Assessment, Planning, Implementation and Evaluation under the state and Nurse Practice Act for Registered Nurse Licensure. Coordinates, manages and monitors the written plan of care for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified and approaches for each. Assists with the coordination, management, and monitoring of the review of nurse’s notes to determine if the care plan is being followed. Assists DON and acts accordingly in the absence of the DON. Participates in nursing call coverage. Works with the Director of Nursing to be available to cover in the event of staff absence and/or tardiness, to maintain regulatory compliance of resident-to-nurse ratio. Monitors resident status changes to ensure appropriate timely nursing or clinical team involvement. Coordinates with department secretary resident MDS and care plan schedule. Oversees non-nursing components of the MDS, Care Plan and CAA’s are accurate representation of the resident and meets regulatory expectations. Functions as a staff nurse when the need arises. Assists in the assessment of quality and safety of care rendered by nursing in the facility. Assists in the development and maintenance of policies and procedures that provide for the care of residents of all ages. Assists, promotes, and maintains harmonious and effective relationship and communication within the nursing department and between nursing and ancillary departments. Assists with the orientation process for all Nursing Services employees. Assists in the evaluation of personal performance and competencies of the Nursing Department, i.e., certified nursing assistants. Assists with in-service requirements of Nursing Department. Understands staffing requirements and assists in maintaining these requirements. Assists in analyzing process improvement data to improve the quality of patient care and the safety of the patients. Attends and participates in continuing educational programs designed to keep abreast of changes in long term care and maintain current RN licensure. Assists in maintaining compliance with state surveys. Assists with and ensures proper and economical use of supplies and equipment for use in the facility to maintain resident care. Collaborates with the medical staff regarding clinical aspects of resident care. Demonstrates knowledge and critical thinking to assess and manage resident’s pain. Adheres to the facility’s resident abuse and neglect policies and procedures. Complies with the unit’s policy regarding reporting of resident changes. Attends units PI and staff meetings.
Two (2) years nursing experience.
Certification & Licensures
RN or LPN licensed in West Virginia required
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required *
