Company: Oak Street Health
Title: Advanced Practice Provider - Nurse Practitioner
Location: Bronzeville Center (4318 S. State St. Chicago, IL 60609)
Oak Street Health believes everyone deserves great healthcare – the kind you’d want for your own family. We specialize in providing exceptional care for older adults on Medicare, often in underserved communities where there is little or no quality healthcare.
Oak Street Health is on a mission to rebuild healthcare as it should be with an innovative care model that centers on wellness-based, positive health outcomes for patients, not the volume of services provided. Our unwavering commitment to keeping patients healthy is highlighted by dedicated care teams that take time to get to know each individual, providing the personalized care they need to stay healthy and live life more fully.
We’re an organization on the move! Our rapidly growing national network of primary care centers is staffed by a diverse team of outstanding care providers, service team members, technologists, community outreach experts, business professionals, and more. For more information, visit http://www.oakstreethealth.com.
Our Care Teams include a doctor, a nurse practitioner or a physician assistant, medical assistants, a scribe, and support from registered nurses and care managers. At Oak Street Health, Nurse Practitioners (NPs) are integral members of our Care Teams. NPs see patients independently and collaborate with doctors, depending upon the patient’s specific situation. NPs provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patient understand and feel connected to his/her care plan.
Together this team is responsible for providing and coordinating care for an intimate panel of patients in our neighborhoods. While the typical primary care panel in the U.S. is around 2,500 patients for a single provider, our panels at Oak Street Health are around 500-750 for a team. This creates an opportunity to spend more time with patients, build deeper relationships, and to better execute/coordinate care plans.
NPs practice in our outpatient practices. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our NPs, in collaboration with our doctors, focus on care within the clinic: primary care, care coordination, and population health.
