Role Description

Oak Street Health delivers high-quality primary care to Medicare patients in neighborhood centers - and at home, when needed. The Mobile Operations Coordinator (MOC) will fulfill a crucial role, supporting logistics, clinical care, and documentation in our innovative home-based care program. The MOC will work directly with the Oak Street Health Home Based Care team and transitional care nurse (TRN) on a daily basis, in collaboration with center-based care teams, to help identify and bridge gaps in care when patients return from the hospital or experience other challenges.


 

Responsibilities:

  • Daily preparation for visit schedule, route, and logistics for all Home Visits

  • Monitoring patient census and managing program referrals

  • Coordinating patient transition back to center

  • Ordering clinical supplies for Oak Street Health home-based care team

  • Complete patient appointment reminder calls

  • Assist transitions nurse with charting and documentation

  • Requesting patient medical records

  • Patient referral coordination and follow up

  • Other duties, as assigned

Position reports to the Director of Clinical Programs

  • Excellent communication, follow-up, teamwork, and problem-solving skills

  • Willingness to learn and be accountable for EMR documentation and workflow

  • Genuine passion for primary care and reaching vulnerable patients

  • Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary)

  • Certified Medical Assistant preferred

  • US work authorization
