Join our team as a Recovery Case Manager and play a pivotal role in providing discharge planning and support to clients leaving the Pacific Hope & Recovery Center (PHRC). Collaborate with a multi-disciplinary team to connect clients with essential local resources, services, and integrated health care, ensuring a smooth transition back to the community.
Primary Responsibilities:
- Collaborate with the multi-disciplinary team at the PHRC and outside referral parties to identify clients' individual needs for efficient and effective discharge.
- Coordinate with referral sources and schedule PHRC admissions.
- Assist clients in accessing entitlements, housing resources, transportation, and outpatient treatment programs.
- Maintain community contacts and partnerships with external providers for streamlined referrals and continuity of care.
- Provide problem-solving and practical assistance to clients in accessing post-discharge resources.
- Collaborate with the client's outpatient clinician, housing coordinator, community agencies, and families to ensure successful discharge plans.
- Collaborate with AOP treatment teams for discharge planning and attend outpatient huddles and collaborative care conferences.
- Support in organizing discharge activities and participate in shift pass downs at the PHRC.
- Adhere to KMHS privacy and security standards for client information disclosures.
Minimum Qualifications:
EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Entry level (no prior related work experience)
This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration.
Preferred Qualifications:
EXPERIENCE: Experienced (minimum 2 years of job-related experience)
Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
The capacity to work well with children and families.
