About the Role:

Astrana Health currently seeking a highly motivated Case Manager. This role will report to a Manager - Case Management and will enable us to continue to scale in the healthcare industry, focusing on supporting the APA ACO SNF population.

What You'll Do:
  • This case manager position is to support the APA ACO inpatient SNF admissions. Currently, there has been an initiative for the HMO IP CM team to support the APA ACO team under the direction of Dr. Patrick Pham for SNF admissions to manage the bed days and average length of stay as well assisting in coordinating the care to our preferred APA ACO providers.
  • The problems that we are bringing this person to solve is coordinating the care to our preferred providers upon discharge as well as monitoring the care of our members during the SNF stay with the support of our SNFist, and also control the bed days and average length of stay of the admission.
  • This person should be successful in directing our APA ACO members to our preferred providers upon discharge along with decreasing the utilization of SNF bed days within the next 60 days
  • The new hire's main daily responsibilities will be to follow the SNF admissions at our contracted SNF for APA ACO members along side our SNFist, will coordinate care upon discharge and direct the discharge needs to our preferred providers (i.e, Home Health, DME etc.). This person will be responsible for initial and concurrent review of all APA ACO SNF admissions.
Qualifications:
  • Active Licensed Vocational Nurse (LVN) or Registered Nurse (RN) license in California
  • Ability to commute to our Monterey Park, CA office
  • Experience using Microsoft applications such as Word, Excel and Outlook
  • Two (2) years of clinical experience as an LVN
  • Skilled Nursing Facility experience a plus

Environmental Job Requirements and Working Conditions: 
  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 1600 Corporate Center Drive., Monterey Park, CA 91754.
  • The total compensation target pay range for this role is: $28.00 - $45.00 per hour. The salary range represents our national target range for this role.
