Job Title: Licensed Vocational Nurse (LVN) Level II
Department: Quality Clinical Operations 
About the Role:
Astrana Health is seeking a dedicated Licensed Vocational Nurse to join our rapidly expanding Dynamic Team. As a Licensed Vocational Nurse Level II, you will play a crucial role in providing direct patient care under the supervision of a nurse practitioner. Your responsibilities will include following the prescribed care plan, administering treatments, and collaborating closely with team members to ensure positive patient outcomes. 
What You’ll Do:
  • Provide direct patient care under the supervision of a nurse practitioner, adhering to the established care plan and treatment protocols.
  • Document patient services accurately and promptly to maintain comprehensive records reflecting the plan of care.
  • Educate patients and their families/caregivers on relevant diagnoses and self-care strategies.
  • Assist in coordinating post-acute care services, communicating identified needs to the interdisciplinary team.
  • Participate in program-specific initiatives, such as Timely Follow Up (TFU) calls and the All-Condition Readmissions (ACR) program.
  • Utilize equipment and supplies efficiently and effectively to ensure optimal patient care.
  • Reinforce hospital discharge instructions and medication adherence with patients, families, and caregivers.
  • Report urgent patient medical issues or conditions directly to the supervising nurse practitioner.
  • Perform tasks including patient blood draws, bone density scans, and glucometer kit teaching, as well as facilitating home visits or telehealth sessions as needed.
  • Engage in personal and professional development activities, including participation in quality assessment and performance improvement initiatives.
  • Fulfill other duties as assigned by the supervising nurse or nurse practitioner.
Qualifications:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Maintains current CPR certification.
  • Minimum of one year of experience in a hospital setting, home health, or community health.
  • Current LVN licensure in the State of California
  • Demonstrates adherence to accepted professional standards and scope of practice.
  • Must possess a valid driver's license, access to insured transportation in good working order, and flexibility to work weekends as needed.
  • Familiarity with the philosophy of Accountable Care Organizations (ACOs) is preferred.

Environmental Job Requirements and Working Conditions:
  • This position will follow a hybrid work structure where the expectation is to work both in office and at home after a 2 week in-person training period. The office is located at 568 W. Garvey Ave, Monterey Park, CA 91754.
  • The total compensation target pay range for this role is: $28-32 per hour. The salary range represents our national target range for this role.
