About the Role:
We are seeking a skilled, compassionate, dynamic Primary Care Nurse Practitioner to propel us forward as we improve quality and access for the 300,000+ members of the community we serve. You will play a vital role in providing exceptional medical care across the community, expanding your impact beyond that of a single medical practice or telehealth-first roles – you will engage in in-person rotations of 3-6 months at our partnered physician practices in order to spread quality, preventive care, and access throughout the community. Your impact will continue to reverberate post-rotation as our physician partners build on your example and best practices to elevate the level of care they provide.

- Partner: Rotate with community physicians in a sub-market of the San Gabriel Valley to deliver in-person, high-quality, preventive-focused and sick care at their clinics
- Lead by example: Care for 15-20 patients per day through Annual Wellness Visits and other preventive services at a partnered physician’s medical practice, focused on quality/HEDIS, accurate condition coding
- Elevate: Collaborate with partner physicians on best practices to improve quality and access at these clinics even beyond rotation
- Innovate: Collaborate with regional and central leadership to propose and launch new programs focused on quality and clinical excellence
Qualifications:
- Unrestricted and active California Nurse Practitioner License
- Current BLS, ACLS certification
- Current California DEA certificate prior to start date
You’re Great for This Role If You Have:
- At least two years of experience working as a nurse practitioner
- Interest in working independently (or have already applied for 103 NP status)
- Experience working in value-based care organizations
- Experience in addressing HEDIS and HCC care gaps
- Internal medicine or primary care experience
- Interest in working with Medicare & Medi-Cal patients
- Proficiency in Cantonese, Mandarin, or Spanish

Environmental Job Requirements and Working Conditions:
- This position is located in Southern California across the San Gabriel Valley.
- This is a 1099 contract position. The initial contract will be valid for 3-6 months. Contracts will be reassessed after the initial term with possible renewal in 3-6 month increments.
- The total compensation target pay range for this role is between $70- $80 per hour. This pay range represents our national target range for this role. Total compensation will be evaluated based on skillsets and experience.
- This is a full-time position.
