About the Role
Clarvida is seeking a Licensed Practical Nurse (LPN) to serve as a THL Care Coordinator, supporting medical and behavioral health services in partnership with a Nurse Practitioner. In this role, you will function in a specialized medical support capacity, assisting with patient appointments, coordinating care activities, and ensuring accurate documentation for individuals receiving prescription medication as part of the Tennessee Health Link (THL) program.
This position plays a key role in maintaining high-quality patient care through thorough recordkeeping, clinical coordination, and direct support during medical appointments.
Responsibilities
● Assist Nurse Practitioner during patient appointments
● Support behavioral health and medical services coordination
● Complete required paperwork, forms, and clinical assessments
● Maintain accurate and detailed patient records, particularly for individuals on prescription medications
● Ensure documentation compliance with program and regulatory requirements
● Provide clinical support related to medication coordination and follow-up
● Perform additional duties as assigned to support program operations
● Support behavioral health and medical services coordination

Requirements
● Active Licensed Practical Nurse (LPN) license in the state of Tennessee
● CPR and First Aid certification, or willingness to obtain prior to working with clients
● Strong attention to detail and documentation accuracy
● Professional verbal and written communication skills
● Ability to work collaboratively within a clinical care team
● Ability to meet background check requirements
