About the Role
Clarvida is seeking an Intensive Case Manager to support individuals with significant behavioral health needs through community-based services. In this role, you will work closely with clients to implement an Individual Recovery Plan (IRP) based on their self-identified goals, strengths, service needs, barriers, support systems, and emotional triggers.
You will provide skill-building, training, and service coordination while linking clients to community resources. Through regular community visits, you will monitor client progress and identify additional supports needed to reduce hospitalizations, incarcerations, and episodes of homelessness while increasing housing stability, employment readiness, and community engagement. This role involves collaboration with medical providers, residential programs, and other service partners to deliver wraparound support.
Responsibilities
● Implement and support Individual Recovery Plans (IRPs)
● Provide skill-building, support, and training aligned with client goals
● Link clients to community-based services and resources
● Conduct regular community visits to monitor progress and needs
● Coordinate care with doctors, residential providers, and service partners
● Support housing stability, employment engagement, and community integration
● Maintain accurate and timely documentation in accordance with program standards
Requirements
● Bachelor’s degree in Psychology, Social Work, or a related field
● Professional verbal and written communication skills
● Knowledge of and sensitivity to culturally diverse populations
● Valid Georgia Driver’s License with a clean Motor Vehicle Record (MVR)
● Clean criminal background check (no conviction or credible evidence of abuse, neglect, or sexual exploitation of children or adults)
