Summary:

At Western Connecticut Home Care, we are seeking individuals to become part of our exceptional home healthcare team. Our home health team works closely with patients’ physicians and families to provide care in the one place where our patients want to be, in the comfort of their own homes. Responsibilities

 

  • Works under the direction and supervision of the licensed Physical Therapist in the home environment practice setting. 
  • Performs Physical Therapy procedures ad related tasks under the direction of the supervising Physical Therapist. 
  • Provides patient/family education and training. 
  • Performs routine operational functions, including superision of Home Health Aide.
  • Documents visits and completes documentation within 24 hours of visit and within agency guidelines.

 Work Type: 

 

Full-Time 

Work Shift: Monday-Friday; 8:30AM-5:00PM

 Location: 100 Saw Mill Road, Danbury, CT

Salary Range

 

22.2500 - 41.3200 (BOE)

 

Credentials:
Essential:
* Cardio Pulminary Resuscitation
* Physical Therapt Assistant License
* Basic Life Support


Education:
Essential:
* Associate's Level Degree

Working conditions:
Essential:
* Significant manual skills / motor coord & finger dexterity
* Some occupational risk
* Extremely heavy effort. May exert > 125 lbs. of force
* Some exposure to dirt, odors, noise, human waste, etc.
