Case Mgmt Social Worker - Per Diem
Description
Purpose: In collaboration with nurses in the Case Management practice, provides patient/family interventions to support clinical, satisfaction, financial, and compliance related outcomes. The Social worker or equivalent addresses the case from a psychosocial-economic perspective by assisting patients and families in adjustment to episodes of acute and chronic illness; in coping with lifestyle changes precipitated by illness or disability; in participating in restorative health programs; in carrying out discharge recommendations; and in addressing issues of end of life care. Using a variety of treatment modalities, assists patients and families in the alleviation or resolution of social, financial and emotional problems related to illness, health care, and rehabilitation. Provides service to patients/customers in the following age categories for Neonatal, Pediatric, Adolescent, Adult and Geriatric.
Essential Responsibilities:
1. Completes an assessment of patients/families then evaluates patient and family information, selects appropriate Social Work methods, develops, implements and evaluates a plan in conjunction with the Case Manager and the multidisciplinary team.
2. Serves as the lead in obtaining financial and other resources for patients/families in need.
3. Actively participates and provides pertinent information in length of stay meetings, discharge planning rounds, during unit daily report, clinical practice teams and department meetings.
4. Initiates patient/family and provider team meetings to develop and plan strategies related to psychosocial and economic issues, particularly those related to care progression and transition. May act as the liaison between patients/families and all providers of care within and external to the organization on social issues.
5. Supports the placement of patients into long term care facilities. Reviews financial resources for long term placement and provides education to the patient/family.
6. Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establishes a plan. Documents interventions and outcomes.
7. Assists in resolving issues in the patient’s environment that interfere with obtaining the maximum benefits from health care.
8. Consults, assesses, refers, coordinates interventions in cases of suspected or actual situations of child or adult abuse and neglect or other forms of domestic violence and/or sexual abuse.
9. Serves as the lead in obtaining legal guardianship and competency determinations as well as adoption related situations. Completes and disseminates all necessary legal and clinical documentation as required.
10. Conducts support groups for targeted populations.
11. Acts as a resource for the BSW staff as required both at Northern Dutchess Hospital and Thompson House.
12. Maintain contemporary professional knowledge and education.
13. Demonstrates regular, reliable and predictable attendance.
14. Performs other duties as required.

Functional/Technical Skills Requirements:
· Health Care and HIT Expertise: Comprehensive understanding of operational health care delivery systems and the current health care landscape. Skills and experience with developing competitive business strategies for health care. Skills and experience with the operational aspects of health care technology deployment. Familiarity and comfort with technology-based operational improvement. Health care management or consulting experience preferred.
· Analytics and Strategy: Expertise in developing and executing data-driven approaches to enhancing business decision-making and improving operational performance (preferably in healthcare). Advanced knowledge of business intelligence best practices, familiarity with fact-based management tools and techniques to drive strategies and a continuous improvement culture.
· Communications: Excellent written and verbal communications skills. Ability to take abstract, complex and/or technical information and break it down for a variety of audiences in a way that is meaningful for them.
· Functional Oversight: Issue identification, gap analysis, ability to prioritize business needs and execute solutions.
· Financial Management: Ability to understand financial reports, develop basic financial models, and identify trends, variances, and opportunities.
Education and Experience Requirements:
· Master’s or Bachelor’s degree in social work or equivalent experience.
· 1 year experience required.
Minimum Knowledge, Skills and Abilities Requirements:
· Must communicate professionally.
· Ability to follow direction and think independently.
License, Registration, or Certification Requirements:
· LMSW or LMHC required
· PREFER: LCSW Preferred
