Kitsap Mental Health Services (KMHS) is seeking a committed and compassionate Population Health Nurse to support our Certified Community Behavioral Health Clinic (CCBHC). In this role, you’ll work directly with clients to complete health screenings, support care navigation, and connect individuals to vital community and medical resources. You will play an important part in strengthening whole-person care by integrating physical health, behavioral health, and social drivers of health across the agency. Primary Responsibilities:

• Conduct health screenings for high-need clients at intake or referral, including vitals, chronic disease history, and social drivers of health assessment.
• Develop brief health plans and provide resource navigation (e.g., food, housing, primary care access).
• Facilitate closed-loop referrals to community providers, ensuring follow-up and completion.
• Provide follow-up encounters to reinforce engagement and troubleshoot barriers.
• Lead monthly group health education sessions for staff and clients as indicated and provide drop-in navigation support.
• Collaborate with psychiatric prescribers and therapists to integrate physical and behavioral health.
• Coordinate with other KMHS staff to perform outreach services in the community and client homes.
• Engage in limited community outreach to strengthen referral networks and partnerships.

 

 

Minimum Qualifications:

EDUCATION: Associate degree in Nursing.

EXPERIENCE: Experienced—two (2) years of experience in population health, primary care, care coordination, or community health nursing.

This position requires driving in a personal vehicle on behalf of the agency; therefore, the incumbent must meet agency driving requirements, including meeting the minimum age requirement of 21 years old, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver's license in the state of Washington, and having reliable, insured transportation.

LICENSURE: Active and unencumbered Registered Nurse license in Washington State. Active and unencumbered multistate registered nurse license accepted with additional training requirements.

 

Preferred Qualifications:

EDUCATION: Bachelor’s degree in Nursing.

EXPERIENCE: Two (2) years of experience working with individuals with mental health and substance use disorders.

Experience working with individuals with complex medical needs.

Experience in providing services to youth and families.

  1. Maintain a working knowledge and skill in verbal de-escalation skills as outlined in Pro-ACT trainings and other agency-mandated trainings
  2. Knowledge of behavioral health populations and social drivers of health.
  3. Knowledge of infection control and nursing standards of care.

  1. Skill in establishing and maintaining effective working relationships with other employees, clients, organizations, and the public.
  2. Strong communication, organizational, and care navigation skills.
  3. Skill in data collection and reporting for population health metrics.
  4. Skilled in accurate and timely completion of documentation in accordance with agency policies and guidelines.

Equipment Operated: Standard office equipment including computers, fax machines, copiers, printers, telephones, etc. Use of a motor vehicle. Use of blood pressure cuffs, thermometers, glucometers, pulse oximeter units, and scales.

 

 

Work Environment: Clinical setting, community outreach, client residences.

 

Mental/Physical Requirements:  While performing the duties of the job, the employee is required to stand, sit, use hands and fingers, reach with hands and arms, ascend and descend stairs, talk or listen and lift up to 50 pounds. Peripheral vision is also required for this position. Frequent mobility and/or sitting required for extended periods. This position requires to ability to react quickly and perceive environmental stimuli accurately, including the ability to physically flee a situation and/or defend oneself against assault.

 

*A sign-on bonus is a form of compensation for a new employee in addition to their established salary. Bonus amounts are prorated by FTE, and any changes to FTE before all bonus installments are paid may alter the total payout.
