About the Role
The Office Care Coordinator is responsible for assisting Nurse Practitioners in providing administrative support and performing clinical functions to ensure optimal services to clients. This role includes delivering approved therapeutic interventions, case coordination, and resource linkage to clients and involved parties to support identified changes within individuals and families.

Responsibilities
· Assist Nurse Practitioners with administrative and clinical functions
· Provide therapeutic interventions, case coordination, and resource linkage to clients and families
· Conduct intake assessments and provide therapeutic services including coping skills, behavior modification, parenting skills, and crisis intervention
· Collaborate with treatment teams to create, personalize, and update treatment plans and documentation in compliance with policies and regulations
· Serve as a liaison and advocate with professionals and agencies involved in client care
· Attend supervision and treatment team meetings and present client updates as required
· Support team members to achieve optimal client and family outcomes
· Complete clinical documentation within required timeframes and correct deficiencies promptly
· Conduct peer audits and contribute to performance quality improvement initiatives
· Assist with patient appointments under supervision of Nurse Practitioner
· Complete paperwork, forms, and assessments for patients receiving prescription medication
· Maintain accurate and detailed patient records

Required Qualifications
· Licensed Practical Nursing License (active or retired) or Bachelor’s degree in a Human Service field
· At least 1 year of experience in the mental health field
· Knowledge of child development and behavior modification
· Cultural sensitivity and ability to work with diverse socioeconomic populations
· Ability to travel approximately 25% of the time
