About the Role
The Licensed Vocational Nurse (LVN) / Licensed Psychiatric Technician (LPT) provides medical and psychiatric support services under the supervision of a physician, nurse practitioner, and/or registered nurse. This role focuses on educating clients on medications, health conditions, and substance use while supporting care coordination for clients with complex medical and psychiatric needs. Responsibilities
• Provide case management and linkage services for clients with complex medical or psychiatric conditions
• Follow up with physicians and pharmacies to ensure prescriptions are filled accurately and promptly
• Administer and dispense medications under the supervision of a medical provider
• Educate clients on medications, side effects, and health conditions including TB, HIV, and hepatitis
• Provide education on substance use and the effects of drugs and alcohol
• Teach parenting skills and child development education
• Establish and maintain relationships with community resources for client support
• Serve as a resource for clients, families, and interdisciplinary team members
• Support care coordination for individuals with multiple diagnoses or medications
• Maintain accurate documentation in compliance with company and regulatory standards
• Ensure compliance with HIPAA, ethical standards, and professional boundaries
• Perform additional duties as assigned

Required Qualifications
• Graduate from an accredited vocational nursing program or psychiatric technician program
• High school diploma or GED required; bachelor’s degree preferred
• Current LVN or LPT license in the state of California
• Valid California driver’s license and active auto insurance
• Minimum one year of acute care or community clinic experience preferred
• Experience with perinatal care and substance use treatment required
• Experience working with severely and persistently mentally ill populations
• Strong organizational, interpersonal, and communication skills
• Intermediate computer proficiency
• Ability to prioritize tasks and manage time effectively
