Job Title: Licensed Practical Nurse – Mobile Medication Services

SUMMARY
The Licensed Practical Nurse (LPN) will be a key member of the Mobile Medication Services team, delivering in-home and community-based medication support to Health Choices-eligible adults with serious mental illness. This role promotes recovery, reduces hospitalization risk, and enhances medication adherence through education, monitoring, and compassionate care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Medication Administration & Monitoring - Administer prescribed medications and monitor for side effects, adherence, and symptom changes. Collaborate with prescribing physicians and psychiatrists to ensure continuity of care.
- Patient Education & Support - Provide individualized education on medication purpose, side effects, and symptom management. Reinforce recovery tools such as Personal Medicine™, Power Statements™, and WRAP plans.
- Clinical Assessment Processing - Conduct ongoing assessments of patient progress, medication tolerance, and overall wellbeing. Document findings and communicate with the broader care team.
- Team Collaboration - Work alongside a multidisciplinary team including RNs, Mobile Medication Technicians (MMTs), Program Managers, and Psychiatrists. Participate in team meetings and contribute to care planning
- Availability & Flexibility - Provide services 24/7, including weekends and holidays, as part of a rotating schedule to meet patient needs.
- Compliance & Documentation - Maintains accurate records in accordance with OMHSAS regulations and Clarvida standards. Ensure PROMISe enrollments and attestation requirements are met.

SUPERVISORY RESPONSIBILITIES
- There are no supervisory responsibilities associated with the Licensed Practical Nurse Medical Medications Services position; however, this person may serve as a coach and mentor for similar positions.
- In absence of the Regional Director/QIDP, Director of Nursing or Residential Program Supervisor, it is the responsibility of the nurse on duty to act as lead staff member in a supervisory capacity to ensure that policies and procedures are followed and that required documentation is completed.
PHYSICAL DEMANDS
- The duties of this job require fine motor skills to write neat, legible notes, read and understand program documentation, and completing keyboarding entry.

WORK ENVIRONMENT
- The work environment for this position varies between residential and community-based settings. Employees are expected to support individuals both in their homes as well as in the community. - The noise level in the work environment is usually moderate.
TRAVEL
- This position requires travel between various residential and office locations. A person in this position may be required to operate company vehicles to transport individuals on outings and/or to and from medical appointments.
REQUIRED EDUCATION AND/OR EXPERIENCE
- Completion of a State approved Licensed Practical Nursing course and possession of a current license to practice as a Licensed Practical Nurse as issued by the Pennsylvania State Board of Nurse Examiners.
- Knowledge of the theories and practice of Licensed Practical Nursing.
- Skill in applying basic nursing practices and methodology.
- Current LPN License valid in Pennsylvania
- Minimum 1 year of experience in behavioral health or community nursing preferred
- Valid Driver’s License and reliable transportation
- Strong communication and documentation skills
PREFERRED EDUCATION AND EXPERIENCE
- Experience with medication-assisted treatment (MAT) or mobile health services
- Ability to work independently and as a part of a mobile team
- Familiarity with HealthChoices Behavioral Health Program and OMHSAS guidelines.
- Comfort with technology for documentation and telehealth support
CERTIFICATES, LICENSES, REGISTRATIONS
- Current license to practice as a Licensed Practical Nurse issued by the Pennsylvania State Board of Nursing Examiners
QUALIFICATIONS
- Language Skills
Ability to read, analyze, and interpret complex documents and instructions. Can showcase the ability to read and understand commonly accepted medical shorthand and jargon. Shows the ability to respond effectively to sensitive inquiries or complaints. Consistently show ability to clearly explain instructions in both written and oral media to non-medical staff members.
- Mathematical Skills
Basic mathematic ability includes addition, subtraction, multiplication, and division.
- Reasoning Ability
Ability to read instructional memos and/or participate in training sessions then translate information provided into day-to-day work duties.
