Registered Nurse Case Manager - Military Readiness
Description
Seeking experienced RN Case Managers to coordinate care for service members processing through mobilization and demobilization. Play a critical role in ensuring soldiers receive appropriate medical care and follow-up.
Responsibilities
- Manage cases for service members with identified medical/behavioral health issues
- Coordinate care between multiple providers and specialties
- Interview service members and review comprehensive medical records
- Implement and monitor plans of care
- Maintain daily unit trackers
- Communicate with commanders and medical staff
- Document all case management activities in MHS Genesis
- See 10-30 service members weekly
Qualifications Required
- Degree: Associate's or Bachelor's in Nursing from ACEN or CCNE-accredited program
- License: Current RN license (any U.S. state)
- Certification: One of the following:
- CCM (Certified Case Manager)
- RN-NCM (Nurse Case Manager)
- CMC (Care Manager Certified)
- Additional: Current BLS certification
- Experience: 1+ year as full-time RN Case Manager within last 2 years
- Citizenship: U.S. Citizen
