Responsibilities:
- Assists in identifying the patient’s physical and environmental needs as evidenced by documentation.
- Provides effective communication to the patient, family, team members and other health care professionals as evidenced by clinical notes, case conferences and communication notes.
- Calls doctor’s offices to obtain and review orders.
- Perform covid tests in-office as needed.
- Works on pre-claim reviews.
- Coordinate care between nurses and patients.
- Assists with field visits as needed.
- Assists with orders management.
- Assists with wellness calls.
- Order supplies.
- Work under supervision of DON.
- Other duties as assigned.
Requirements
- LVN and CPR certified required
- Home Health experience required
- Strong computer and data entry skills
- Prior QA experience a plus
- Able to multitask, strong sense of urgency, attention to detail
- Job Knowledge:
- Comprehensive knowledge of the function and organization of the home health agency. Knowledge of
- current rules and regulations in Home Care. Ability to facilitate quality control issues.

decisions; understand and solve problems within limits of authority; work under tight deadlines and
maintain accuracy of work.
