Reliant at Home is seeking a Full-Time RN Hospice Admissions Nurse to cover Fort Worth and surrounding areas. This opportunity has an expectation of 2 admissions a day working from 10AM-7PM, Monday through Friday. 

Hospice experience preferred. Homecare Homebase EMR experience a Plus!

Responsibilities
  • Primary functions are to perform admissions and recertifications.
  • Evaluation Assess hospice patients using the Comprehensive OASIS Assessment or other appropriate assessment to identify the physical, psychosocial, and environmental needs.
  • Develop and document the plan of care in correlation with the assessment.
  • Provide skilled nursing care using infection control measures (OSHA). 
  • Provide supervisory visits to the Aide and /or LVN/LPN to ensure quality and continuity of service provided. 
  • Recertify hospice patients using the Comprehensive OASIS or other appropriate assessment.
  • Timely submission of all required paperwork. 
  • Administer skilled nursing care for clients of all ages in their place of
  • Coordinate care with the interdisciplinary team, patients and their families, and referring agency, and assume the responsibility for coordination of care.
  • Demonstrate commitment, professional growth and competency. 
  • Meet productivity standards. 
  • Provide effective communication to patients and their families, team members, and other health care

Requirements:

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Current State license as a Registered Nurse, current State Driver's License.
  • Two years experience as a Registered Nurse in a clinical care setting and 1 year full time hospice experience.
  • Reliable transportation. Valid and current auto liability insurance.
  • Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills preferred in palliation of end-of-life. Proof of current CPR. Must read, write and comprehend English.
