Provides direct patient care services, as well as, administration, clinical supervision, documentation reviews, hiring, education and evaluation for social workers and social services functions of the acute care hospitals across the district. Consults on social work issues and serves as subject matter expert and resource expert to Palomar Health. Provides oversight for the development and coordination of case management/discharge plans for patients in collaboration with physicians, other interdisciplinary staff, and appropriate community-based resources/healthcare professionals. Maintains working knowledge of national and community standards and scope of social work practice. Responsible for planning and participation in the provision of in-services to other medical and nursing staff regarding the social and emotional components of illness and social work programs. Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, Powerpoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.
