The responsibilities of the HIMS Record Specialist include Document Imaging and Release of Information. Staff performs functions to safeguard and protect the patient’s rights of privacy ensuring accuracy of their medical record and ensuring authorized individuals have access to the patient’s medical information. Additionally, each associate is to maintain a professional health care office environment on day to day basis by maintain a clean work space, and a professional and positive image of the department and its practices. This includes maintaining confidentiality by keeping all patient and medical information seen and heard in the facility in the strictest confidence. Work with all staff members to promote a developmental and cordial work environment.

Responsibilities

DOCUMENT IMAGING:

1.Commitment to perform all job responsibilities in accordance with the philosophy and mission statement of LMH Health.

2.Honest, ethical, professional behavior is a condition of employment.

3.Walk throughout the facility/clinics to pick up the necessary paper medical health information to ensure timely and accurate electronic medical documentation.

4.Performs scanning of the paper health information record into the electronic health record following established guidelines from designated departments/clinics within the organization.

5.Performs qualitative and quantitative analysis of scanned document images to assure accuracy and legibility maintained.

6.Promptly scans and indexes records received through alternative means, such as faxing, mail, etc.

7.Ability to work in conjunction with a team to complete the daily workflow timely, accurately and effectively.

8.Participates with clinical staff and supervisors to establish well-coordinated compliant scanning routines.

9.Promotes an atmosphere that encourages enthusiasm regarding accurate record maintenance and availability.

10.Ability to operative and communicate effectively while under pressure.

11.Maintains work schedule flexibility to meet short timeframes.

12.Confirms accuracy of all patient and document information throughout the document imaging process in order to ensure quality of all patient information in EMR.

13.Performs at established standards of performance as set by the Health Information Management Director.

14.Informs the Supervisor, in his/her absence, Director of HIMS, of all issues, complaints, incidents, etc. immediately.

15.Provides back-up coverage for releasing of medical records and merging documents into the electronic health record.

16.Maintains current knowledge related to applicable statutes, regulations and standards necessary to perform the job.  Participates in the development and updating of training materials.

17.Informs the Supervisor, in his/her absence, Director of HIM, of all issues, complaints, incidents, etc. immediately

18.Strong ability to handle multiple priorities with frequent interruptions

19.Performs other duties as assigned.

RELEASE OF INFORMATION:

3.Compliance with the request, authorization, company policy and HIPAA statutes.

4.Interacts with requestors to identify medical record needs for patient care, review, billing or release of information via phone or in person.

5.Requires strong critical thinking skills and performs self-quality checks on all work to assure accuracy of the release.

6.Prioritizes release of information requests based on procedure, STAT procedures and HIPAA guidelines.

7.Accesses computerized patient record and/or other computer systems for patient information for other requestors of the medical record.

8.Handles all requests & inquiries for patients whether received via mail, fax, phone or in person.  Verified identity & confirms that the authorization is valid.  Ensures the requesting party has a legal right to request a patient’s medical record.

9.Calculates invoices & manages accounts receivable for records that have been invoiced and waiting on payment.

10.Performs at established standards of performance as set by the Health Information Management Director.

11.Informs the Supervisor, in his/her absence, Director of HIM, of all issues, complaints, incidents, etc. immediately.

Qualifications

Required:

  • High school diploma or equivalent.
  • Advanced computer knowledge and experience with software programs.
  • Excellent interpersonal and communication skills.
  • Must be knowledgeable regarding patient health information and HIPAA compliance.
  • Must demonstrate the ability to be self-directed.
  • Must demonstrate the ability to follow the organization’s confidentiality and security rules.

Preferred:

  • 1-3 years of healthcare work experience.
  • Associate's degree.
  • Certificate in Health Information or related field.
  • Experience with document imaging or releasing of medical records.
