Who We Are Looking For:

Are you the Bilingual Community Health Worker we have been looking for?  In this position you will be a medical and social advocate by working with our clients! 

PACT Program provides intensive, home-based care to people living with HIV and Hepatitis C who are having challenges navigating their healthcare and treatment. In the PACT Program, the Community Health Worker (CHW) supports people living with HIV, Hepatitis C, as well as other chronic illnesses and help them access the benefits and resources they need.

What You’ll Do:

  • YOU visit clients in their home on a regular basis.
  • YOU will provide adherence-focused education, skills coaching, outreach, and accompaniment to individuals living with HIV/HCV;
  • YOU will connect with medically underserved individuals through culturally responsive outreach and provide them with education, accompaniment, and resources.
  • YOU will help schedule, remind, and accompany clients to regular medical/social appointments.
  • YOU will work with clients to identify personal health goals and action steps towards achieving those goals. 
  • YOU will assess client’s social and structural needs and assisting them in obtaining the services to meet these needs.
  • YOU will establish a therapeutic and trusting relationship with each client and assist the client as they follow the treatment recommendations of their doctors, practice healthy behaviors, and obtain necessary support services.
  • YOU will assist with the development and maintenance of collaborative relationships with local substance use treatment centers, HIV service organizations, health clinics and social service providers.
  • YOU will actively represent PACT/JRI services in a professional and positive manner.
  • YOU will document all participant interactions in the client record; this includes writing progress notes, renewing chart documents when they're due, and designing service plans for each participant.
  • YOU will attend regular supervision with the PACT Program Manager; attend weekly team meetings and other staff meetings as expected; perform other related tasks as assigned.

Requirements:

  • Must be 21 years or older.
  • Fluency in Spanish and English required.
  • Applicants must have 1 - 4 years of case management or community health worker experience; OR a Bachelor's degree in related field.
  • Candidates with lived and/or professional experience with people living with HIV/HCV, other chronic diseases, mental health issues, and/or substance use preferred.
  • Candidates must have an active driver’s license and vehicle.
  • Candidates must demonstrate respect for people living with HIV/HCV/substance use disorders and their diverse lifestyles and choices.
  • Creativity, flexibility, sound judgment, and ability to take initiative.
  • Strong communication and writing skills.
  • Excellent time management and organizational skills.
  • Demonstrated ability to work as an effective team member in a complex and fast-paced environment.
  • Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and clients.
