Job Summary
We are seeking a dedicated behavioral health professional to provide Supported Employment and Housing Services for members with Serious Mental Illness (SMI). Responsibilities include:
- Supporting members in obtaining and maintaining employment following SAMHSA’s Supported Employment Core Principles.
- Assisting in housing support and placement through Housing First/Permanent Supportive Housing models.
- Conducting intakes, assessments, and individualized case planning.
- Coordinating with the clinical team and housing specialists to secure community-based resources.
- Managing financial assistance funds for housing needs.
Minimum Qualifications
- Education & Experience: HS Diploma/GED + 4 years of behavioral health experience OR a Bachelor’s degree (preferred).
- Licensure: Must obtain an Arizona Fingerprint Clearance Card within 8 weeks of hire.
- Transportation: Reliable vehicle, valid AZ Driver’s License, and state minimum insurance coverage.
