Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
Community Options, Inc. licenses Adult Development Home Providers (ADH) and Child Development Home Providers (CDH). This is your chance to help make a difference by opening your home to an individual with developmental and/or physical disabilities. This independently contracted position will provide a 24/7 family-like setting for an individual.
The Developmental Home Provider will ensure that best practices and technology are used to teach the individuals how to acquire the physical, intellectual, emotional, and social skills needed to live as independently as possible.
REQUIREMENTS:
  • At least 21 years of age, pass a background check, and obtain a Level One Fingerprint card
  • Adequate transportation
  • At least 1 year of previous work experience as a caregiver
  • Willing to complete 22 hours of training through Community Options (CDH Providers are required to complete an additional 15 hours of Foster Parent College training)
  • Provide 3-character references
  • Provide paystubs for at least 1 month
  • Complete an annual physical
  • Pass the State of Arizona Life Safety Inspection
  • Complete the interview process with a licensing specialist
  • Submit all required documents for the licensing process
  • Must be open to supervisory visits in your home, including unannounced visits
Development Home Providers are contracted and reimbursed at a daily habilitation rate, PLUS they receive monthly room and board from the individual they support – this income is tax-exempt.
