We are searching for a Community Habilitation Support Specialist to join our agency.
A non-management position, which is responsible for assisting, providing, and promoting independence in individual’s daily support needs, care, and provision of services. Com Hab Support Specialist’s will provide support and assistance to the entire team. They will assist the CSBA/CSPM to ensure all agency and regulatory compliance is maintained. Com Hab Support Specialist will respect each individual’s personal choice, lifestyles, and their dignity to take informed risks.
Responsible for the implementation and maintenance of various records and ensuring that the program and individuals have their needs, wants and services provided to them in the highest quality and manner.
To assist the management and clinicians with the monitoring of implementation and/or development of each individual’s plans of service (IPP, ISP, ICP, RESHAB, IPOPO, Life Plan or Treatment plan, etc). Assist with staff and individual training, and assist in the follow up of direct care staff as it relates to the implementation and documentation of but not limited to, habilitation data collection, communication logs, health care concerns, recreation logs, behavior sheets, ADL sheets.
- High School Diploma/GED required
- Must have a valid NYS Driver’s License in good standing
- Minimum of 1 year of related experience, specifically in Human Services
- Must be at least 18 years old
- Must be able to lift a minimum of 50 pounds
