Licensed Clinical Social Worker - Emergency Department - FTE - Evening Shift

Salary Range: $58.11 - $78.46 plus applicable shift differential
The ED Social Worker functions as a member of the interdisciplinary healthcare team to facilitate efficient patient movement as well as manage behavioral health emergencies and other case management issues, beginning in the Emergency Room through the continuum of care. Interfaces with internal departments as well as external departments. Responsibilities include discharge planning for complex patients in the ED to prevent inappropriate admission for patients that do not meet medical necessity criteria by procuring appropriate services and resources. The ED Social Worker is also responsible for behavioral patients’ assessments in order to determine the need for a 5150. The ED Social Worker shares responsibility for responding to a Code BERT. The ED Social Worker oversees consults with mother and baby patients requiring screening before discharge. The ED Social Worker can be assigned other duties and responsibilities as seen fit.
- Masters in Social Work
- Licensure as LCSW
- Three to five years of work experience in a health care setting
- Acute hospital experience preferred
- Knowledge of post-acute care community resources and referrals
- Medical terminology knowledge
- Basic computer skills required
- Demonstrates effective interpersonal and communication skills
- Demonstrates flexibility via an ability to adapt to changing priorities
- Demonstrates good customer relations
- Ability to prioritize assignments and effective time-management skills
- Basic knowledge of clinical and psychosocial aspects of patient care
- Must be detail oriented, flexible and committed to patient advocacy
- Demonstrates skills in planning, organizing and managing multiple functions and complex processes
- Excellent verbal and written communication skills required
- Knowledge of basic computer software programs
