We are hiring an Assisted Living & Memory Care Nurse Manager who is responsible for managing and responding to the needs of residents, ensuring quality care, implementing and overseeing the programming, and service delivery for Assisted Living and Memory Care. This position manages licensed nursing staff and non-licensed staff. This position ensures dementia support and programming extends to those in independent living. What You Will Do: 
  • Establishes and maintains Assisted Living policies and procedures, in collaboration with the
    Director of Wellness or Director of Nursing and the Executive Director or Administrator, based
    on the evolving needs of Sequoia Living Assisted Living and Memory Care residents.
  • Plans and provides direct nursing care to residents, assigns and schedules nursing responsibilities to staff.
  • Conducts rounds on all floors of Assisted Living and Memory Care for quality assurance.
    Conducts monthly clinical audits.
  • Develops, reviews and analyzes reports including but not limited to assessment, intervention
    implementation, physical and chemical restraint reduction, falls, weight change, dietary and
    pharmacy considerations, medical needs and behavior stamps for physician review, and
    communicates results and implementation programs to appropriate personnel. 
  • Completes and monitors necessary forms for physical and chemical restraint assessment and
    reduction. 
  • Communicates regularly with physicians, residents, families, administrative personnel, and
    appropriate staff regarding residents’ physical and mental condition and resident care needs.
  • Responds to emergency and specialized resident care situations, evaluates nursing care practices and communicates information regarding the resident for proper care and care related concerns to appropriate personnel, residents, and responsible parties.
  • Updates and maintains resident care plans, as well as resident roster and census. 
  • Ensures resident needs are met, partnering with activities and other support services as necessary. 
  • Verifies correct medications, dosage, and administration compliance based on pharmacist audits.
  • Remains current on dementia care approaches and makes recommendations around additional training, and/or resident program changes or additions.
  • Represents the Assisted Living and Memory Care services in the Quality Assurance Performance Improvement (QAPI), by ensuring program and nursing domains are monitored and remedial activities are explored. This includes recording QAPI activities and reporting to the QAPI committee, as necessary. 
  • Collects and analyzes data, establishes new procedures to ensure correction of recognized care issues; reports findings at QAPI meetings. 
  • Reviews physician orders, nursing notes, and progress notes for compliance with state and federal regulations. 
  • Interviews, hires, supervises, trains, motivates, and provides performance management of
    assigned staff.
  • Stands in for the Director of Wellness or Director of Nursing in their absence, for
    interdisciplinary team, resident care, and other required meetings. 
  • Manages all risk management duties pertaining to the AL/MC including investigating, reporting, and communicating incidents.
  • Partners with the Director of Life Enrichment to supervise volunteers, with activities such as
    reading to residents, as needed.
  • Facilitates resident moves from Independent Living to the Health Center, from Assisted Living to the Health Center and from Independent Living to Assisted Living.
  • Adheres to legal and ethical standards related to resident confidentiality, advance directives, and informed consent. Participates in quality improvement initiatives, regulatory surveys, and
    accreditation processes.
  • Performs other work as assigned.

  • Knowledge of admissions criteria.
  • Knowledge of Matrixcare, RL6, RCFE regulations within Title 22 in regard to the provision of activities and social services, basic audio-visual skills, and Microsoft Office Suite. Experience with time keeping systems. Ability to learn new applicable software programs. 
  • Knowledge of dementia care best practices and techniques. Compassionate and empathetic approach to working with seniors and individuals with memory impairment. Ability to prioritize and manage multiple tasks in a fast-paced environment. Proficiency in electronic health record systems. 
  • Previous experience working with individuals with memory impairment in a healthcare of senior living setting. Physical skills and ability to perform work that requires continual standing, walking, stooping, bending, and lifting of at least 50 pounds
  • RN or LVN licensure from an accredited school of nursing to practice in the State of California. Minimum five (5) years nursing experience, with one (1) year minimum experience in Assisted Living or Memory Care and/or Skilled Nursing. Two (2) years supervisory or management responsibility.
