Description

We are hiring a Resident Resource Coordinator to join our growing team. You will be responsible for enhancing the quality of life of Tamalpais Assisted Living and Health Center residents including the transition periods between levels of care by providing individual attention, encouraging socialization and identifying appropriate service providers. What You Will Do: 
  • Recognizes residents’ needs, provides individualized attention to residents and directs attention to the most isolated. Initiates contact with residents who receive the least attention from others.
  • Matches residents of like interests, plans interactive activities and encourages participation in organized activities.
  • Works with groups of no more than 3-4 residents, developing conversation groups, arranging games, reading, and providing entertainment through media.
  • Reads to one or more residents, arranges for books on tape, keeps residents informed of community events, writes letters and reads mail, assists residents in making phone calls and in making purchases.
  • Assists residents in completing forms such as the Resident Assessment Tool; contacts families to assist with the Resident Assessment Tool if the resident is incapable.
  • Takes residents on walks, accompanies them to the main dining room and takes them to community events.
  • Acts as a companion, especially to those who have few contacts and encourages residents to interact with others.
  • Calms and/or distracts residents who are agitated. Promotes independence and educates in independent skills.
  • Refers residents to outside resources such as day care programs, counseling services or Love is the Answer (LITA) where appropriate.
  • Provides direction/suggestions to private duty personnel.
  • Recognizes safety and comfort issues, addresses and/or refers to appropriate departments. 
  • Contact families and friends where appropriate.
  • Assists those moving from Independent Living to the Health Center, from Assisted Living to the Health Center and from Independent Living to Assisted Living.
  • Performs other work as assigned

Who You Are: 
  • You enjoy working with people and have exceptional interpersonal skills. You can use tact and courtesy in dealing with residents under sometimes trying conditions. You can show patience and understanding. You enjoy variety in your day and enjoy a fast-paced environment.
  • Knowledge related to the issues of aging and resident care. Proficient with Microsoft Office suite of products (Outlook, Word, Excel, PowerPoint, Publisher), charting and documentations for residents.
  • Proficient written and verbal English communication skills to complete necessary reports and follow instructions in order to communicate with residents and their families. Effective interpersonal skills to meet and greet residents and their families in a positive manner.
  • Compassion for ability to work with older persons with special needs. Ability to be cooperative with fellow employees and accept their suggestions where appropriate. Ability to attend training functions specifically for this position and general training as required of all staff. Demonstrated selfmotivation, self-confidence, flexibility. Ability to be firm and develop relationships. Ability to maintain positive customer focused approach and confidentiality of resident, staff and organization information at all times. Ability to act with patience, tact and courtesy under demanding and difficult conditions. Sensitivity and understanding of issues related to aging.
  • High school graduate or equivalent. Current Certified Nurse Assistance (CNA) Certification, Bachelor’s degree in Social Service or Activities preferred. Minimum 2 years’ experience as paraprofessional in Assisted Living or Skill Nursing Facility. Current CPR and First Aid Certifications.
