Description
Responsible for the overall maintenance and control of all patient medical records. Handles requests from doctors, pharmacies and patients. Scans and file charts, faxes records, and delivers various messages to appropriate party. Ensures medical records are safely transported. Delivers messages and incoming faxes to appropriate staff. Organizes paper files/charts/loose documents/radiological scans and prepares all for scanning. Electronically file labs, x-rays, patient information, pharmacy requests, reports, consultations, etc. Analyzes medical records for quantitative deficiencies and forwards completed records for processing. Checks all pages in record to verify correct patient information. Communicates with physicians and other departments to facilitate release of information. Conducts periodic audits of medical records to ensure compliance with standards. Contacts physicians to expedite the completion of medical records and obtains patient authorization to release records as required. Ensures that institutional policies and procedures for maintenance of medical records are followed. Handles problem requests for medical information. Interprets and implements policies relating to disclosure of confidential patient medical information. Processes and closes patient files, including final verification of all information. Analyzes and answers correspondence requests in compliance with applicable policies, procedures, and regulatory requirements/statutes. Assist in training/orienting department personnel as needed. Transports medical records to and from multiple storage facilities as needed. Travel to other offices as needed for medical record projects.

Ability to speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position.  Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, PowerPoint, internet, e-mail).  Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop-down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.  Performs other duties as assigned.
