Description
Wage Range: $42.19 - $77.81 per hour
Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
EvergreenHealth is pleased to offer our PER DIEM Hospice Home Care RN’s option and choice when it comes to their work assignment. We believe that creating a tiered system for our per diem RNs allows for flexibility and balance, honoring the employee’s desire to work while balancing given business needs. See below for details. Further information will be provided at time of interview.
Tier 1 Per Diem RN
- Work 5 days per month, including
- 2 weekend days included in the 5-day minimum
- 17% hourly premium
Tier 2 Per Diem RN
- Work 7 days per month, including
- 3 weekend days included in the 7-day minimum
- Availability for 1 holiday annually (paid)
- 20% hourly premium

- Physicians-The patient’s Attending Physician continues to provide oversight of care and Hospice Physicians consult with hospice staff and the attending physician.
- Nurses (RN or LPN)

- Certified Nursing Assistants (Home Health Aides)
- Spiritual Care (Chaplains)
- Pharmacists
- Trained Volunteers- offer companionship for patients and respite for families

Job Summary:
EvergreenHealth Hospice Home Care is committed to providing the highest quality of care to our patients in both King and Snohomish counties. Hospice clinicians visit patients where they live – private homes, assisted living, adult family homes and skilled nursing facilities – providing care and support to the patient and their family unit during their end of life journey.

Primary Duties:
- Provides direct patient care.
- Assesses the comprehensive nursing needs of an assigned group of patients (including, but not limited to home care patients).
- Coordinates the patient and family/significant other education process.
- Initiates patient start of care through established admission process.
- Continuously plans, implements, and evaluates patient care, incorporating patient and family/significant other into the process.
- Participates with physician partners and other interdisciplinary team members to develop and implement patient and family/significant other plan of care.
- Communicates effectively all relevant information (e.g., patient information, safety issues, problems with equipment, quality issues, etc.) to immediate supervisor, in timely way.
- Communicates and documents changes in patient condition, on a timely basis, to team members, DPOA, and physician.
- Delivers nursing care within established standards of care, hospice, hospital and unit guidelines, policies and procedures.
- Functions as a patient advocate.
- Assists other staff in completion of their duties, when time permits.
- Recognizes situations when appropriate assistance is needed to effectively execute the nursing process.
- Promotes good public relations between patient, family members, hospital, and community.
- Practices within the scope of the Registered Nurse Practice Act.
REQUIRED for the position:
- Bachelor's in Nursing (BSN) required within 5 years of employment if applicable
- Licensed as a Registered Nurse in the State of Washington
- Current Healthcare Provider BLS certification by date of hire
- Current Washington State Driver's license and proof of insurability

The literature shows that having a Bachelor of Science in Nursing (BSN) contributes significantly to the quality of care. Toward that end, this position requires a BSN or a commitment to begin a BSN program within three years and obtain a BSN within five years of employment. Please note this BSN requirement does not apply to those with an RN license for twenty or more years or current EvergreenHealth RNs hired prior to April 1, 2014.
