Wage Range: $26.91 - $43.05 per hour
Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
Job Summary:
The Care Management Coordinator performs a variety of duties supporting population health and clinical care management initiatives. Responsible for care management related administrative activities including, but not limited to, patient education and outreach, follow-up phone calls for disease management, identifying patient care gaps, abstracting clinical data, retrieving relevant outside medical records, referral management, and utilization review. Functions as a communication link for the Care Management staff, providers, insurance companies, departments, and other agencies. Works with clinical providers and other key stakeholders by supporting data collection processes and making recommendations to improve data collection methods, accuracy, and reporting. Participates in performance improvement activities in support of department and organization goals.
Primary Duties:
- Engage patients with care management services, and other community resources as directed by the clinical staff and other health care professionals.
- Conduct follow-up phone calls, electronic messaging, or mail letters to target populations(s) as directed.
- Schedule patients with RN Care Managers and Primary Care Providers as directed.
- Review patient lists and disease registries to identify patients with high and rising-risk conditions.
- Conduct outreach to rising and high-risk patients to eliminate identified care gaps.
- Obtain outside medical records to reconcile patient medical records withing scope.
- Work withing multiple EMRs and portals.
- Document data and care in the population health care management system according to privacy and confidentiality standards and regulations.
- Prepare, review, and audit the data collected for accuracy and completeness.
- Proactively seek opportunities to improve abstraction and reporting efficiencies and identify non-clinical tasks withing the scope of the role.
- Provides administrative support for Care Management Team including word processing, data entry, filing, organizing, and medical record management.
- Respond to requests regarding patients, including medical review agencies, insurance companies, healthcare team and community resources.
- Completes special projects within scope of training and experience with the direction and assistance of the Director, Manager, or designee.
- Practice fiscal stewardship to ensure appropriate utilization of resources.
- Collaborates with clinic providers and care teams in the delivery of care management functions and therapy plans. May require routine, local travel to EvergreenHealth clinics to facilitate rounding, communication, and other related needs for patients and clinic staff.

License, Certification, Education or Experience:
REQUIRED for the position:
- High School graduation or equivalent
- Current license as LPN in the State of Washington; or current WA State Medical Assistant Certification and current national certification through the American Association of Medical Assistants (AAMA), American Medical Technologist (AMT), National Healthcareer Association (NHA), American Medical Certification Association (CMAC), or National Center for Competency Testing (NCCT); or commensurate clinical experience
- 2 years of experience in health care, acute care hospital or ambulatory clinic
- Experience with electronic medical record (EMR) system
- Must maintain license as appropriate to profession, if applicable
- Current Washington State Driver’s license and proof of insurability
- Reliable transportation
- Knowledge of medical terminology
- Experience with quality metrics to include HEDIS and STARS
- Ability to interact with others in a professional and courteous manner
- Excellent oral and written communication skills
- Exceptional organizational skills
- Proven ability to effectively and proactively complete multiple projects simultaneously and meet deadlines
- Strong attention to detail, accuracy, and ability to work independently
- Exhibits dependability and good judgement skills
