Who We Need
We’re looking for someone with the following skills, experience and credentials:
As a member of the Integrated Care Team, the Case Manager Trainee will assist in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress.
Assist with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the Integrated Service Plan are implemented.
Assist in adjusting the ISP and service delivery as needed based on progress/lack of progress in the specified health, wellness and behavioral health goals.
Assist in maintaining, monitoring and modifying covered behavioral health and healthcare services and community resource needs;
Other duties as assigned.
Job requirements
Must be enrolled in, or graduated from an Bachelor’s program in Human Services, Psychology, or related field and available to attend required trainings and consistent work schedule that meets program needs. Must at least have a High School Diploma/G.E.D
Desired career path in field of direct services for behavioral/integrated healthcare provider.
Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements. Safety sensitive is any job that includes tasks or duties that the employer in good faith believes could affect the safety or health of the employee performing the task or others
