Starting Pay: $18.50-$22.50
Job Summary
As a member of the Integrated Care Team, participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.

Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.
Who We Need
We’re looking for someone with the following skills, experience and credentials:
- Works collaboratively with the integrated care team to engage, educate, communicate, and
coordinate care with service participants, their family, behavioral health, medical and dental
providers, community resources and others in ensuring that all services prescribed in the
Integrated Service Plan are implemented. - Provide or arrange for transportation, as appropriate and determined by the care team and
provide services in the community. - Completes documentation of services and meets member engagement levels according to the standards of Copa Health.
- Develops and maintains and manages the Integrated Service Plan (ISP) within required
timeframes. - Adjusts the ISP and service delivery as needed based on progress/lack of progress in the
specified health, wellness, and behavioral health goals. - Provides assistance in maintaining, monitoring, and modifying covered behavioral health and
healthcare services and community resource needs. - Maintains the person’s comprehensive clinical record, including documentation of activities
performed as part of the service delivery process (e.g., assessments, provision of services,
coordination of care, discharge planning). - Coordinates and communicates with Clinical/Program Directors, Clinical Coordinators, Integrated Care Coordinators, nursing, and medical and psychiatric practitioners for individuals exhibiting urgent or emergent medical and/or psychiatric symptoms.
- Participates in regular care team meetings and staffing and documents actions to modify service delivery based on progress, health care activation, current gaps in care, the Health Risk Assessment and level of self-management of chronic conditions.
- Provides continuous screenings and evaluations of treatment effectiveness for service referrals, including counseling, residential and supportive services, and community resources. Makes recommendations for initial and concurrent authorization of service referrals. Participates in and supports fidelity of integrated care practice, including training, performance measurement and enhanced communication protocols for Integrated Care teams.

Minimum requirements
- Bachelor’s Degree in Psychiatric Rehabilitation, Rehabilitation, Social Services, Psychology, or a related field plus two (2) years of direct care experience OR
- HS Diploma/GED and six (6) years of full-time direct care experience.
- Minimum of one (1) year of experience providing employment/rehabilitation-related services and support to individuals with disabilities required.
- Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
- TB test clearance required at the time of hire.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire
- Bilingual (English/Spanish) speaking preferred.
