ABOUT THE ROLE
Are you passionate about improving health outcomes and working in a collaborative, value-based care environment? Join our dynamic team as an Integrated Care Coordinator (ICC) and help bridge the gap between behavioral and physical health. The ICC plays a key role in managing high-risk patients, supporting clinical performance goals, and ensuring exceptional care coordination across disciplines.
You’ll work side-by-side with primary care providers, behavioral health professionals, case coordinators, and population health teams to improve outcomes for members with complex needs. This is a highly collaborative, hands-on role focused on achieving measurable results and enhancing quality of care.

WHAT YOU’LL DO
  • Support Clinic Leadership and population health teams in meeting Value-Based Care initiatives.
  • Analyze and track Value-Based Measure (VBM) data to identify performance gaps.
  • Engage directly with members to improve outcomes, particularly those on the High-Risk Registry.
  • Assist clinical teams in refining workflows and identifying process barriers.
  • Participate in morning meetings, huddles, and staff collaboration to improve care delivery.
  • Provide brief, evidence-based interventions when appropriate (e.g., motivational interviewing).
  • Carry a short-term caseload of complex members as needed.
  • Document interactions in the Electronic Medical Record (EMR) in a timely and accurate manner.
  • Coordinate with Care Management to address causes of high healthcare utilization.
  • Support treatment plan modifications in collaboration with clinical teams.
  • Maintain communication standards including email and call response within 24 hours.
  • Comply with agency policies, clinical protocols, and licensure requirements.

Minimum Requirements:
  • Bachelor's degree in behavioral health + 1 year of direct care experience
    OR
  • High school diploma/equivalency + 5 years of behavioral health direct care experience
  • Valid AZ Driver’s License, reliable transportation, and current auto insurance
  • Eligibility for AZ Department of Public Safety fingerprint clearance
  • TB Test clearance at time of hire
  • Bilingual in English/Spanish preferred
