Starting pay $21 DOE
Who We Need
This job is responsible for working within a multi-disciplinary care team to provide rehabilitation services to participants that include a focus on gaining competitive employment and/or engaging in psychosocial rehabilitation activities or educational goals. Rehabilitation and Employment Specialists are responsible for developing and sustaining a culture of work at the Health Home as an integral part of recovery. You will develop relationships with community employers, providers, and other organizations to foster an increase in employment and meaningful community activity opportunities for members.
MINIMUM QUALIFICATIONS:
  • Bachelor’s Degree in Psychiatric Rehabilitation, Rehabilitation, Social Services, Psychology, or a related field plus two (2) years of direct care experience OR
  • HS Diploma/GED and six (6) years of full-time direct care experience.
  • Minimum of one (1) year of experience providing employment/rehabilitation-related services and support to individuals with disabilities required.
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
  • TB test clearance required at the time of hire.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire
  • Bilingual (English/Spanish) speaking preferred.

JOB SUMMARY (Including, but not limited to):

  • Works collaboratively within a multi-disciplinary care team to provide services geared towards employment and meaningful community activities
  • Participates in daily care team morning meetings and works collaboratively with the care team to identity members who are in the hospital, incarcerated, etc, who could benefit from meaningful community activites (i.e. day programs, IOPs, WAT, ERS etc.) ]
  • Engages with members in the community including hospitals, jails, and residential treatment settings to complete referrals to meaningful community activities/psychosocial rehabilitation to support in transitional care planning.
  • Manage a flexible caseload as determined by the Clinical Coordinator including a mix of approximately 2/3 employment related rehabilitation and approximately 1/3 psychosocial rehabilitation; the caseload size is determined based on a dynamic review of both the clinical need and resources available.
  • Provide and track detailed information on all employment related engagement and caseload.
  • Utilize psychiatric rehabilitation best practices including evidence-based employment practices to assess participant’s needs emphasizing a Copa Health Work First philosophy for appropriate individuals to reach their desired outcome.
  • Educates and assists participants in identifying the resources needed to obtain and maintain employment including groups and workshops at the Campus.
  • Provides limited job coaching virtually, within the health home, and in the community until services for job coaching can be obtained by an employment provider agency.
  • Utilize available tools to screen and assess participants for employment and psychosocial rehabilitation opportunities, e.g., VAP, MCAW, readiness assessment, vocational profile, etc.
  • Identify opportunities for participants that are externally and internally supported through employment networks.
  • Coordinate care and collaborate with RSA staff, vocational and rehabilitation providers, and other stakeholders to ensure preferred and most effective services are provided to members.
  • Under the Vocational and Rehabilitation Services Interagency Governmental Agreement (IGA), works in partnership with assigned Rehabilitation Services Administration (RSA) Vocational Rehabilitation Counselor(s), community rehabilitation program providers and others involved in assisting members to become involved in or maintain involvement in meaningful community activities or employment.
  • Maintains expertise and fulfills RSA collaborative inter-governmental agencies' training requirements.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
