About the Role
We are seeking a Rehabilitation Specialist to join our multi-disciplinary care team. In this role, you will support participants in reaching their goals around competitive employment, psychosocial rehabilitation, and educational opportunities. You will play an essential role in fostering a culture of work and community involvement as part of each participant’s recovery journey.
This position works directly with participants in the community—hospitals, jails, residential treatment centers, and beyond—to connect them with meaningful opportunities. You’ll collaborate with community employers, providers, and agencies to build partnerships and increase access to employment and rehabilitation services.
Key Responsibilities
- Provide individualized employment and psychosocial rehabilitation support within a multi-disciplinary care team.
- Develop relationships with employers and community organizations to expand opportunities for participants.
- Engage participants in transitional care planning and referrals for community-based services.
- Manage a flexible caseload (approximately 2/3 employment-related and 1/3 psychosocial rehabilitation).
- Conduct assessments and use evidence-based practices to support participant goals.
- Deliver limited job coaching and employment readiness support.
- Maintain accurate documentation, data tracking, and required reporting.
- Collaborate with the Rehabilitation Services Administration (RSA), vocational providers, and other stakeholders.
- Educate and support case managers and team members on referrals and employment-related services.
Qualifications
- Bachelor’s degree in Psychiatric Rehabilitation, Rehabilitation, Social Services, Psychology, or related field plus 2 years of direct care experience
OR - High School Diploma/GED and 6 years of full-time direct care experience
- At least 1 year of experience in employment/rehabilitation services supporting individuals with disabilities
- Valid AZ Driver’s License, reliable transportation, and ability to meet Copa Health driving requirements
- TB test clearance at hire and ability to obtain AZ Fingerprint Clearance Card within 8 weeks
- Bilingual (English/Spanish) preferred

- Proficient in Microsoft Office and comfortable learning EHR/EMR systems
- Strong interpersonal and communication skills (written and verbal)

- Ability to build and maintain professional, supportive relationships
