Position Summary
Copa Health is seeking a Care Treatment Coordinator to facilitate coordination of care for members on all health plans. Ensures implementation of services to members in accordance with their health plan contractual requirements and reviews progress.  Coordinates with involved healthcare, behavioral health and wellness providers, while maintaining current documentation on members’ progress and coordination of care efforts. Manage medical and psychiatric conditions, while assisting member in achieving their recovery goals.
Key Responsibilities
  • Accepts referrals from sites or Member Engagement Team for new members.  Works directly with assigned Psychiatric and Primary Care team to engage, educate, communicate, and coordinate care with service members, their family, behavioral health, medical and dental providers, community resources, health plan liaisons and others to ensure that all services in the Integrated Service Plan (ISP) are implemented. 
  • Updates and maintains required medical record documentation. 
  • Responsible to maintain an abbreviated Case Manager (Supportive/ Connective Level of Care) assigned caseload.
  • As warranted by the health plan: Completes an annual Integrated Comprehensive Assessment (ICA) and updates it, as necessary.
  • Facilitates Coordination of Care with all health plan liaisons
  • Ensures the development of discharge and aftercare plans prior to discontinuation of behavioral health or primary care services.

Qualifications

  • Bachelor’s degree in behavioral health related field and 2 years’ of full-time behavioral health work experience; or 
  • Bachelor’s degree in non-behavioral health related field and at least 4 years’ of full-time behavioral health work experience; or High school diploma or high school equivalency diploma and 6 years’ of full-time behavioral health work experience
  • Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
  • Valid AZ Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company’s driving requirements and safety sensitive requirements.
  • TB Test clearance required at the time of hire and annually.
