MINIMUM QUALIFICATIONS:
Requires any combination of training, education, and experience listed below:
- Minimum HS diploma/GED and two (2) – three (3) years of relevant experience in a behavioral health setting; OR
- An associate degree and one (1) - two (2) years of relevant experience in a behavioral health setting; OR
- A Bachelor’s degree (related or non-related) in the behavioral health field plus one (1) year of relevant experience in behavioral health.
- Required to maintain a valid Arizona Driver’s License and can pass a motor vehicle screen for insurance purposes in order to transport clients.
- Experience in working with multi-cultural and low socio-economic individuals.
- Must have a Level 1 Fingerprint Clearance Card through the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)
- Ensure the well-being of the individuals served and provide a positive, supportive and structured environment.
- As applicable; assist, teach, coach, and mentor individuals in skills of daily living to include but is not limited to housekeeping, personal hygiene and grooming, budgeting, shopping, community activities and community resources.
- Engage individuals served in activities and interactions designed to encourage achievement of treatment goals.
- Ensure the daily completion of goals identified in the persons Treatment Plan, Behavior Plan, or Vision Recovery.
- Demonstrate skills in planning, implementing, and coordinating health promotion strategies including health and wellness activities.
- As applicable; assist in the planning and preparation of menus consisting of well-balanced meals and meeting applicable diet needs as required.
- Document timely, accurate an appropriate clinical information in the individuals’ clinical record.
- Maintain billable hourly requirements as assigned.
