Job Summary: This position will provide support for the Assertive Community Treatment team at the assigned Health Home.

Essential Job Duties and Responsibilities:

  • Creates and maintains consumer charts including filing, thinning, closing, etc. Completes ROI packets as requested. Assists with the processing of PCP notifications, medical record requests, and refill requests.
  • Supports the Morning Meeting and weekly Integrated Care Meeting protocols through management of the meeting logs and daily assignment schedules.
  • Manages the team referral status roster, including collection of baseline data.
  • Coordinates and monitors scheduling for the assigned team. Responsible for maintaining Prior
  • Authorizations for services and assists medical staff with medication Prior Authorizations.
  • Acts as a liaison between team and consumer (greets, receives, and escorts consumer to appointment
  • when needed).
  • Provides back up Blue Dot coverage in a customer service capacity of screening calls and taking
  • messages as needed.
  • Meet member engagement goal as defined by Copa Health, by documenting billable services in the
  • medical record.
  •  Monitors and updates ACT member eligibility including screening for AHCCCs eligibility and supporting
  • ACT members to apply for and maintain their benefits
  •  Assists the Clinical Coordinator in auditing charts for completed paperwork in the medical record. Updates the Clinical Coordinator and ACT Specialists when completed paperwork is needed or in need of an update.
  • Assists team in tracking participant schedules by utilizing calendar of activities for the team to assure that team is meeting the consumer’s needs.
  • Completes reminder calls to the consumer’s regarding their appointments in the clinic.
  • Participates in fidelity self-monitoring and compiles all needed information on ACT tracking sheets to assist Clinical Coordinator with monitoring Fidelity Measures for the team.
  • Assists Clinical Coordinator with managing ACT staff schedules, vacations, and coverage for scheduled
  • hours and weekends.
  • Manages Bus Pass assignment and ordering process for the ACT team.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.


MINIMUM QUALIFICATIONS

  • Must be a minimum of 21 years of age.
  •  Minimum HS diploma/GED and two (2) years of relevant experience in a behavioral health setting.
  •  Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
