Who We Need

This position is located at our Administration Office in Mesa AZ.

The Care Manager is a clinical support to the integrated care team, responsible for supporting and coordinating the behavioral health and physical health care of High Cost/High Need members per an assigned caseload (minimum 90 members). The Care Manager coordinates the overall effort of the care team and ensures effective communication among team members including the ability to create a tailored plan for who will complete each of the tasks.

MINIMUM QUALIFICATIONS

  • Requires a Masters in Nursing, Social Work, Counseling or related field,
  • Must have an Arizona professional license at the associate (LAC or LMSW) or independent level (LPC, LCSW or Clinical Psychologist).
  • Minimum of 4 years’ experience in behavioral health or healthcare to include direct care management, utilization management, medical management and or population health.
  • Proficiency in typing; using computer software and Internet is essential.
  • Experience with screening for common behavioral, medical and/or substance abuse disorders.
  • Experience with assessment and treatment planning for common behavioral, medical and/or substance use disorders.
  • Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
  • Develops, implements, supports and promotes integrated health strategies, tactics and programs that support the delivery of quality healthcare to members.
  • Utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate physical and behavioral healthcare and social services for members through assessment and member-centered care planning, direct coordination/collaboration with treating providers and specialists, and coordination of psychosocial wraparound services to promote effective utilization of available resources.
  • Serves in a consultative role to care coordinators, case managers, psychiatric and medical staff on initiatives within the outpatient sites, including identification and assignment of participants to a care management panel and consultation on evidence-based strategies for clinical, medical, family, environmental and social interventions, as applicable.
  • Through the use of clinical tools and review of member specific health information/data, conducts comprehensive and ongoing assessments of referred member's needs/eligibility and, in collaboration with the member's care team, determines an approach to resolving member issues and/or meeting needs by evaluating the available internal and external programs/services and resources.
  • Applies clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex clinical indicators which impact care planning and resolution of member issues
