JOB SUMMARY (Including, but not limited to)
  • Collaborates with inpatient and outpatient care teams to assess needs and develop plans that support long-term health and reduce re-admissions.
  • Acts as a resource to primary case managers to enhance engagement and ensure understanding of post-discharge medical instructions.
  • Coordinates specialized post-discharge placements (e.g., skilled nursing, home health, residential).
  • Communicates with RBHA, inpatient/outpatient teams, families, and supports to implement discharge plans.
  • Completes and submits discharge documentation and referral packages in a timely manner.
  • Coordinates post-discharge appointments within 72 hours and ensures follow-through on all recommendations.
  • Identifies and addresses service gaps; ensures medication, transportation, and follow-up supports are in place.
  • Tracks and facilitates service referrals based on the discharge plan.
  • Travels as needed for discharge staffing, appointments, record retrieval, and community-based visits.
  • Serves as the point of contact for CPRP and hospital admission/discharge alerts through the health information network.
MINIMUM QUALIFICATIONS
  • High School Diploma / GED or associate's degree with a combination of eight (8) years’ direct care experience, required.
  • Bachelor’s degree in behavioral health or related field and three (3) years direct care experience, preferred.
  • Previous experience in behavioral health or primary care with duties of case management and care coordination, preferred.
  • Demonstrated experience and knowledge in discharge planning and readmission prevention processes, preferred.
  • Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety of eligible to receive within eight (8) weeks of hire.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
  •  Demonstrated ability to communicate, problem solve, and work effectively with people and in a team  
  •  Excellent organizational/time management skills with the ability to manage multiple priorities.
  •  Strong assessment and motivational interviewing skills.
  •  Ability to take initiative and see tasks to completion.
  •  Knowledge of SSI, coordination of benefits, and insurance benefits.
  •  Demonstrate empathy for their members (patients) and work cooperatively to develop a plan the
  •  member/patient is likely to follow.
  •  Demonstrate dependability, reliability, and professionalism.
  •  Maintain professional relationships and appropriate boundaries with participants, service providers, and
  •  internal and external customers.
  •  Respond to emails and returns phone calls within 24 hours or the next business day.
  •  Maintain cooperative and effective workplace relations with members of the patient centered treatment team.
  •  Comply with required workplace safety standards
  •  Comply with company’s incident reporting procedures.
  •  Maintain confidentiality and adherence to HIPAA rules at all times.
