Who We Need
In this position of Housing Specialist, you will perform professional work by coordinating housing services to an adult population composed of persons with SMI, substance-abuse disorders, dual-diagnoses and/or referred persons. You will work with tenants, clinical teams, landlords, payees, guardians and community stakeholders. Implement quality supports and services which promote person-centered planning and clinically appropriate practices including the following principles for serving individuals enrolled in the Behavioral Health System, including the utilization of Evidenced Based Practices (EBP) as directed.
MINIMUM QUALIFICATIONS:
Requires any combination of training, education, and experience listed below:
- Required HS Diploma/GED plus four (4) years of full-time direct care experience in behavioral health.
- Bachelor’s degree highly preferred.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
- Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)
- Conducts assessments, determines eligibility, and develops individualized case plans.
- Develop Individualized Service Plans for each participant using the comprehensive assessment.
- Links individuals/families with community-based housing (utilizing the Housing First/Permanent Supportive Housing), EBP.
- Educates and utilizes community resources.
- Assist with budgeting techniques using the members financial funds.
- Provides case management services in conjunction with community partners.
- Is responsible for Program development, monitoring, and evaluations/fidelity.
