Who We Need

We’re looking for someone with the following skills, experience and credentials:

  • Collaborates with care team and Hospital Discharge Specialist to identify individuals in need of housing and housing support services. Provides education, coordination and resources to care team and service participants in the range of housing options available, ensuring maximum choice to the extent possible.
  • Serves as campus knowledge expert and point of contact on housing issues, options, housing providers and supportive service agencies, including:

    • Access to furniture resources and start-up kits.
    • Housing options for difficult to place and legally involved individuals.
    • Language barriers and physical mobility issues in community housing.
    • Landlord tenant rights and responsibilities.
    • Permanent Supportive Housing Evidence Based Practice model.
    • RBHA housing tools including SPDAT and housing application process.
  • Tracks individuals placed on housing waitlists and ensure care team follow up as needed to secure and maintain housing.
  • Participates in daily team meetings as well as any additional individual treatment teams or other team communication meetings as required. Participates in hospital staffing's and housing intakes as well as any additional discharge meetings pertaining to housing as needed.
  • Provides direct housing support services as needed and trains care team members in supportive services delivery and meeting individual needs to ensure housing stability (e.g. eviction prevention, food boxes, furnishings). Documents all member events and services in accordance with policies, procedures, and clinical protocols
  • Assists members/family or and coordinates with clinical care team in assisting members/family in the choosing, locating, obtaining, and keeping appropriate housing.


Job requirements

  • High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health or
  • Bachelor’s Degree in a related field OR a bachelor’s degree in a non-related field with one (1) year experience preferred.
  • Must have 1 year of training or experience in vocational rehabilitation and support. Prior experience may be job development, job coaching, or supported employment.
  • Must have a Level One Fingerprint Clearance Card by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • TB Test clearance required at the time of hire.
