Role and Responsibilities:

The Case Manager (CM)/Community Supports Coordinator (CSC) is responsible for the assessment and care management of persons enrolled in the Developmental Disabilities and Supports Waiver programs and functions in a collaborative team role as an integral member of the Interdisciplinary Team (IDT). This role will provide case management services and will set the tone for creating a comfortable atmosphere in the IDT so that the person(s) with their voices or other advocates can support the concerns of the person(s). The person we are serving is why these systems exist, and therefore, why it is so important that we listen and support the person to achieve their desired goals.

 

Knowledge/Skills/Abilities:

  •           The CM/CSC is responsible for the Initial Waiver Eligibility and Waiver Enrollment Services stage.
  •           Conducting a program meeting annually.
  •           Educating the participant regarding the required documentation and submission process to establish Medical Eligibility and monitor the status of the submission of the required documentation to the Income Support Division (ISD).
  •           Ensuring the initial and annual evaluation of the Level of Care (LOC) is complete.
  •           Initiating pre-planning meetings and assists the participant to identify or develop a circle of support.
  •           Working with the participant through pre-planning meetings and an ISP meeting during the participants budget year, to create an ISP and budget.
  •           Completing a formal, ongoing monitoring process to evaluate the quality, effectiveness, and appropriateness of services and supports provided to the person as specified in the ISP.
  •           Contacting the participant at least monthly to discuss items identified on the DD and Supports Waiver contact form.
  •           Additional compensation available for bi-lingual/Spanish speaking candidates 

Required Experience:

  •           Have one-year clinical experience, related to the target population, working in any of the following settings:

         Home health or community health program

         Hospital

         Private practice

         Publicly funded institution or long-term care program

         Mental health program

         Community based social service program

         Other programs addressing the needs of special populations, e.g., school.

  •           Or have a minimum of 6 years of direct experience related to the delivery of social services to people with disabilities.
  •           Must have a valid driver’s license that is in good standing.
  •           Each successful applicant will complete a criminal history background check resulting in no disqualifying offenses.
  •           Each applicant will also be required to complete all state required pre-service training to qualify as a CM/CSC.

Education:

  •           Hold a license as a social worker, as defined by the NM Board of Social Work Examiners; OR
  •           Possess a bachelor's or master's degree in social work, psychology, counseling, nursing, special education, or closely related field, OR
  •           Hold a license as a registered nurse as defined by the NM Board of Nursing.
