Essential Duties: The Psychiatric Hospital Tracker will consult with individuals served by the CA Regional Center, service coordinators and caregivers concerning individual’s mental health issues and subsequent hospitalization; communicate with hospital personnel concerning individual’s admitting diagnoses, treatment and discharge through phone calls and hospital visitations; participate in discharge planning meetings; consult with the regional center psychiatrist, mental health liaison, psychologist and service coordinators concerning post hospitalization placement and follow-up; assist with finding outpatient mental health services for discharged individuals; assist with coordinating follow-up care for individuals served and circle of support; document all activities on virtual chart and monthly reports; provide other related services as requested by supervisor.
While performing the duties of this job, the employee is regularly required to speak and listen with prolonged periods of sitting at a desk and working on a computer. The employee is regularly required to stand, walk, sit, and use hands to handle devices or use writing tools. The employee frequently lifts and moves up to 10 pounds.
Qualifications:
Education:
- Masters’ Degree in psychology, social work, or marriage and family therapy.
License:
- License: Licensed Social Worker in the State of CA or licensed RN in the State of CA.
Experience:
- 2 years of experience as a Licensed Social Worker
- Experience interacting with mental health personnel and agencies.
- Applicants must have experience working with individuals with developmental disabilities.
- A psychiatric nurse would also be eligible to apply.
