Clinic LVN Float
Description
The Position: Under the supervision of the RN Supervisor, the Licensed Vocational Nurse (LVN) provides nursing services to participants at the CEI day center. The LVN communicates effectively with all members of the day center team and other program staff to ensure that the participants are receiving care that is appropriate and follows the provider orders and the participants’ plan of care developed by the MDT.
The rate range for the Clinic LVN position at Center For Elders Independence is $ 30.10 to $ 45.15 per hr. Pay rate based on the market for the Clositiinic LVN position, as well as experience, skills, abilities and work history.
**Please note: This is a fulltime, fully-benefited "Float" position. The person holding this role will not work at the same Center every day and will "Float" to other Centers as needed**
DUTIES AND RESPONSIBILITIES:
- Provides nursing services to participants as ordered by primary care providers in a timely manner within the scope of the ADHC.
- Administers, monitors and records prescribes medications and treatments, including medications, oxygen etc. as ordered.
- Coordinates and implements appropriate health-screening programs for participants.
- When assigned, participates in MDT meetings and assists in the development and maintenance of the participant’s plan of care.
- Notifies the providers of any marked changes in participants’ condition, including discovery of any wounds, any acute physical or behavioral changes.
- In conjunction with RN initiates follow-up services and provides care coordination as needed.
- Provides teaching, counseling and health education to the participants and family members regarding nursing care needs.
- Initiates preventative and rehabilitative nursing procedures as appropriate for the participants’ care and safety.
- Contacts consulting physicians for participants as necessary.
- Responsible for supervision of personal care services provided by center aides and completion of daily personal care flow sheets as delegated.
- In conjunction with RN assists with emergency response and arranges for emergency services such as emergency transportation.
- Participates in department and agency meetings.
- Completes all required paperwork as assigned.
- Participates in all agencies quality improvement program (QI) as assigned.
- Maintains the confidentiality of all company procedures, results and information about participants or families.
- Follows all CEI Policies and Procedures.
- Complies with all agency training requirements
- Maintains safe working environment by following CEI’s safety P&P’s.
- Maintains a courteous, helpful and professional attitude on the job. Displays a willingness and ability to be responsive to all customer groups
- Performs other duties as assigned in a positive and helpful manner.
QUALIFICATIONS:
- Graduate of a school of professional nursing.
- Have a current California Vocational Nurse License.
- A minimum of one (1) year acute experience, and experience working with the frail or elderly population.
- Excellent telephone /customer services skills
- Exercises initiative and flexibility.
- Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public.
- Maintains eligibility for licensure by obtaining required continuing education units.
- Excellent interpersonal skills.
- Well organized
- Ability to effectively problem-solve complex issues.
- Strong verbal and written communication skills
- Ability to work as a team player in a multi-cultural, multi-disciplinary setting.
- Have a current CPR and First Aid Certification
