THE POSITION

Reporting to the Director of HCBA, the HCBA RN Case Manager works closely with a team of RNs, Patient Care Navigators and Social Workers to develop a plan to allow applicants to live safely in the community. 

The salary range for this role is $93,850-$140,750.  The salary for HCBA RN Case Manager is based on the market for the position, as well as experience, skills, abilities and work history.

DUTIES AND RESPONSIBILITES
•    Perform intake assessments and regular re-assessments of program participants through a combination of telehealth and home visitation
•    Assess level of care needed to allow participants to live safely in their homes
•    Assess and implement necessary waiver service levels
•    Regularly evaluate service level effectiveness and safety
•    Assess non-waiver service levels and connect clients to additional service agencies
•    Perform monthly check-ins with participants
•    Approve treatment authorizations and verify service delivery 
•    Complete home safety evaluations
•    Assists with development, review and revision of departmental policy and procedures which support positive outcomes and organizational objectives.
•    Open to best practice model suggestions.  
•    Offer input and comply with department standards of care and practice as well as quality improvement activities
•    Develop collaborative, productive relationships with healthcare facilities and partner agencies to achieve timely discharges that include optimal participant care and outcomes across the continuum
•    Investigate participant concerns as identified and offers satisfactory resolution regarding case management role
•    Models ethical behavior; adheres to organizational policies and guidelines; maintains confidences; admits mistakes; conducts work with honesty; shows consistency in words and actions and follows through on commitments
•    Demonstrate openness to creative and innovative approaches to providing service.
•    Comply with all DHCS reporting requirements


QUALIFICATIONS:
•    Graduate of accredited school of nursing. Current California Nursing RN license required 
•    CPR certificate required
•    3-5 years of Case Management experience preferred
•    Case Manager Certification preferred
•    Understanding of Nurse Practice Act and all other local, state, and federal regulations
•    Demonstrated strong interpersonal communication skills
•    Experience working with electronic medical records and intermediate/advanced computer skills (Microsoft Office Suite) required
•    Ability to perform duties in heavy workload, fluctuating environment
•    Bilingual: Spanish/English or Cantonese or Mandarin/English, preferred
