Community Health Worker

The Community Health Worker (CHW) is a trusted member of the community in the delivery of health and support services at BAART Community HealthCare’s two San Francisco Clinics; BAART Market and BAART Turk. The CHW will increase a client’s health, health knowledge and self-sufficiency through a range of activities such as outreach, engagement, informal counseling, social support, escorts to appointments and advocacy. The Community Health Worker will also assist patients/clients with identifying eligibility and health care coverage, PCP assignment and obtaining/retaining access to the medical and mental health services at either of the BAART Clinics.

The CHW will participate with various trainings provided BAART and by the San Francisco Community Clinic Consortium (SFCCC).

Responsibilities:

  • The Community Health Worker (CHW) is a trusted member of the community in the delivery of health and support services at BAART Community HealthCare’s two San Francisco Clinics; BAART Market and BAART Turk. The CHW will increase a client’s health, health knowledge and self-sufficiency through a range of activities such as outreach, engagement, informal counseling, social support, escorts to appointments and advocacy. The Community Health Worker will also assist patients/clients with identifying eligibility and health care coverage, PCP assignment and obtaining/retaining access to the medical and mental health services at either of the BAART Clinics.
  • The CHW will participate with various trainings provided BAART and by the San Francisco Community Clinic Consortium (SFCCC).
  • Collects data and information as required TBD.
  • Provides feedback to program management.
  • Uses assigned technology to complete job functions effectively.
  • Maintain and document confidential information.
  • Attends required meetings with SFCCC CHW Pilot Program Cohort (approximately monthly while onboarding and quarterly after).
  • Submits quarterly report to SFCCC on program implementation, data of individuals receiving assistance, and client stories sharing highlights, challenges, and recommendations.
  • Performs other job duties as required by manager/supervisor.

Qualifications:

  • Minimum age requirement of 18.
  • Community Health Worker Certification or willingness to become certified within 1 year of hire.
  • Trained in use of program EHR’s; primarily Practice Fusion and Avatar, may include Methasoft.
  • Trained and capable to teach basic health, sanitation, hygiene matters within 6 months.
  • Trained in basic concepts of health promotion and disease prevention within 6 months.
  • Trained in basic behavior change strategies within 6 months of hire.
  • Experience in community work, education, health care, or related field preferred.
  • Demonstrated self-awareness in terms of understanding one’s own race, culture, identities, biases, power, privileges, and stereotypes.
  • An understanding of the diverse academic, socioeconomic, cultural, disability, gender identity, sexual orientation, and ethnic backgrounds present in the community. Sensitivity to populations most highly affected by homelessness and other at-risk populations.
  • Must be able to read, write, and speak English. Fluency in other languages is an asset.
  • High School diploma or its equivalent is preferred.
  • Understands and is part of the community being served.
  • Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner.
  • Strong interpersonal, communication and organizational skills. Solid aptitude for customer service.
  • High degree of skill in grammar, spelling and punctuation.
  • Ability to maintain the confidentiality of all information obtained.
  • Strong organizational skills and the ability to prioritize workflow.
  • High level of analytical problem-solving skills.
  • Must be action-oriented and able to work independently and as a team player.
  • Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite, Outlook.
  • Satisfactory references from employers and/or professional peers
  • Satisfactory criminal background check
  • Satisfactory drug screen

Salary Range:

The salary range for this position is $28.00 to $30.77 an hour. 

The salary of the candidate(s) selected for this role will be set based on a variety of factors, including but not limited to, experience, education, specialty and training.
